Family Health Partnership Clinic provides high quality healthcare for the uninsured in McHenry County. The Clinic operates on a sliding fee scale, although no patient is turned away due to inability to pay. At registration, patients will be asked to provide some proof of income.

The Community Care Connection is our mobile health vehicle that travels throughout Lake County, offering residents a variety of free health screenings, including blood pressure, blood sugar, cholesterol, body mass index (BMI), osteoporosis screenings and foot exams. Screenings are offered free of charge to adults aged 18 and older, and appointments are not necessary.

Low Cost Health care and dental coverage.
Illinois’ All Kids program offers health care coverage to children or helps in paying premiums of employer or private health insurance plans. All Kids services are available at no cost or at low cost. Premium and co-payments are determined based on your family income and size. All Kids Share and Premium Level 1 are jointly funded by the state and federal governments. All Kids Rebate and Premium Level 2 are state funded. All Kids Assist is the Medicaid portion of Illinois’ health care programs for children.

The Access to Care (ATC) program is administered by the Suburban Primary Health Council. ATC is a non-profit primary health care program for residents of suburban Cook County and northwest Chicago who are lower-income, uninsured, and underinsured.
