Northwest Community Healthcare

This clinic provides preventive and restorative oral health services to individuals who have an income at or below 200 percent of the federal poverty level or are on General Assistance or Medicaid.

Eligibility and Fees

The townships’ General Assistance staff determines treatment eligibility for residents of Elk Grove, Palatine, Wheeling and Schaumburg townships. A $30 co-pay is charged for most adult appointments, and there is no co-pay for children. For more information or to qualify for the program, please contact your township:
