The Community Care Connection is our mobile health vehicle that travels throughout Lake County, offering residents a variety of free health screenings, including blood pressure, blood sugar, cholesterol, body mass index (BMI), osteoporosis screenings and foot exams. Screenings are offered free of charge to adults aged 18 and older, and appointments are not necessary.

The following services are available free of charge:

• Blood pressure measurement
• Blood sugar testing (diabetes screening)
• Body mass index (BMI)
• Cholesterol testing
• Flu shots
• Osteoporosis screenings (bone density testing)***
• Referrals as needed for additional healthcare services or community resources
• Sports physicals for children and teens****
